TRAVEL REQUEST FORM
Travel must be approved by the treatment court team and submitted
no later than the Friday before your phase report date.
Applications received for dates that occur before your next report date willnot be reviewed unless there are extraordinary circumstances (death in the family, etc.).

All terms and conditions of probation remain in effect
while you are traveling, including drug and alcohol testing.
You will need to contact the local District Court
or Police Department before you leaveto arrange for your testing needs.
You are responsible for providing documentation of out-of-towntesting to the Specialty Court upon your return.
You will be responsible for the payment of this testing.
